Online Therapy: How It Works, When It's Recommended, and What the Limits Are

More and more people, especially in Florence and neighboring towns, ask me if online psychological therapy really works and whether it's suitable for them too. The short answer is: it depends on the situation. In this article I explain how an online path works, who it is truly suited for, and in which cases, instead, an in-person meeting is preferable.

How an online therapy session works

An online psychotherapy session takes place via video call, with a frequency and duration similar to an in-person meeting (generally 45-50 minutes). Before starting an online path, the psychologist is required to provide clear informed consent: the technology used, privacy protection measures, the communicative limits of the digital medium, and the available alternatives, including in-person ones. Even remotely, the therapeutic relationship remains subject to professional secrecy and the same ethical rules as a physical office.

Who online therapy is truly suited for

Based on my clinical experience, the online format works well for several situations:

  • Those who live outside Florence or in nearby areas and cannot easily reach the office on a regular basis
  • Out-of-town students or people who have recently moved
  • Those who work inflexible hours and need to fit the session into their lunch break or evening
  • Psychological support paths, stress management, mild-moderate anxiety, relationship or couple difficulties, low self-esteem
  • Those who have already started an in-person path and need to temporarily continue remotely (travel, relocation, health reasons)

✅ When online therapy is recommended

  • You live outside Florence or in nearby areas
  • You are out of town or have recently moved
  • You have inflexible work hours
  • Mild-moderate anxiety, stress, relationship difficulties, self-esteem

⚠️ When in-person office visits are better

  • Risk of self-harm or suicidal ideation
  • Severe or decompensated psychiatric disorders
  • Eating disorders in acute phase
  • Emergency situations (in these cases: emergency room or 112)

When an in-person office visit is preferable (or necessary)

Online therapy has real limits, and it's right to state this clearly rather than presenting it as a universal solution. It is generally not recommended as the sole intervention in situations of:

  • Risk of self-harm or suicidal ideation, which require a more structured evaluation and care, even in person
  • Severe or decompensated psychiatric disorders, which require close clinical monitoring, often in collaboration with a psychiatrist
  • Eating disorders in the acute phase, which often require a multidisciplinary team
  • Emergency situations in general: in these cases the correct reference is the nearest emergency room or your local emergency services

Frequently Asked Questions

Is online therapy as effective as in-person therapy?

Psychotherapy research shows encouraging results for many common issues (anxiety, stress, relationship difficulties, support for personal growth), when the format is chosen appropriately for the problem and the person. It is not automatically an equivalent alternative in every case: this is why the initial assessment remains essential.

How does the first online session work?

As with a first in-person consultation: we get to know each other, I understand what brings you to seek help at this time, and together we assess whether the online path is the right choice for you, or whether an in-person meeting in Florence is preferable.

What tools are needed to get started?

All you need is a stable internet connection, a webcam, and a private space where you can speak freely, without the risk of being overheard.

Yes: professional secrecy applies online exactly as it does in the office, and I use tools designed to protect the confidentiality of communication, which I discuss transparently at the first contact.
