You weren&#8217;t dreaming, or at least you don&#8217;t remember anything. You were simply asleep, and an instant later you were gripped by terror.<\/p>\n\n<p>Gli <strong>nocturnal panic attacks<\/strong> are panic episodes that arise during sleep and wake the person up in full activation. Those who experience them almost always describe them as more frightening than daytime ones, and there&#8217;s a specific reason for that.<\/p>\n\n<h2>Why they&#8217;re more frightening than daytime attacks<\/h2>\n<p>During the day, a panic attack almost always has a context: you were on the subway, you had a meeting, you were in line at the supermarket. Even if the reaction is disproportionate, the mind finds something to attach it to.<\/p>\n<p>At night that context doesn&#8217;t exist. You wake up already inside the attack, with no before, with no apparent reason, disoriented by the darkness and the abrupt shift from sleep to waking. And the absence of a cause is exactly what makes it easier to think: \u201cso it&#8217;s my heart,\u201d \u201cso I&#8217;m really dying.\u201d<\/p>\n<p>On top of this, there&#8217;s the loneliness of the hour. At three in the morning there&#8217;s no one to call, and the perception of threat is amplified.<\/p>\n\n<h2>They are not nightmares, and they are not night terrors<\/h2>\n<p>This is the most useful distinction to make, because they are different phenomena that require different responses.<\/p>\n<p><strong>The nightmare<\/strong> occurs during REM sleep, that is, the part of sleep in which you dream. You wake up remembering the content of the dream, and the fear has a story: someone was chasing you, you were falling. The nocturnal panic attack does not: there&#8217;s no dream to recount.<\/p>\n<p><strong>Night terror<\/strong>, or pavor nocturnus, mainly affects children. The person seems awake, screams or is agitated, but is actually not, and in the morning remembers nothing. In a nocturnal panic attack, however, you genuinely wake up and remember everything with great clarity.<\/p>\n<p>Nocturnal panic attacks typically emerge from the phases of <strong>non-REM sleep<\/strong>, generally during the transition into deep sleep, often in the early part of the night. This is why there&#8217;s no dream material associated with it: you weren&#8217;t dreaming at all.<\/p>\n\n<h2>The symptoms<\/h2>\n<p>They are the same as those of a daytime panic attack, but experienced from a different starting condition:<\/p>\n<ul>\n<li>Sudden tachycardia, heartbeat perceived as irregular or very strong<\/li>\n<li>Air hunger, feeling of suffocating<\/li>\n<li>Sweating, trembling, chills or hot flashes<\/li>\n<li>Chest pain or tightness<\/li>\n<li>A feeling of unreality, of not recognizing the room<\/li>\n<li>Fear of dying or losing control<\/li>\n<\/ul>\n<p>The episode reaches its peak within a few minutes and then subsides on its own. What lingers for a long time, often until morning, is the adrenaline surge: the trembling, the difficulty falling back asleep, the mind that keeps racing.<\/p>\n\n<h2>First of all: rule out medical causes<\/h2>\n<p>This step should not be skipped, especially with the first episodes.<\/p>\n<p>Several organic problems can cause awakenings with similar symptoms: <strong>sleep apnea<\/strong>, gastroesophageal reflux, thyroid disorders, drops in blood sugar, cardiac arrhythmias, some medications and the discontinuation of others. Apnea in particular is common and often undiagnosed: those who suffer from it wake up with a start because their breathing has actually stopped.<\/p>\n<p><strong>Talk about it with your doctor<\/strong> and let them decide whether tests are needed. Knowing that you&#8217;ve ruled out physical causes isn&#8217;t just clinical prudence: it&#8217;s also what allows you to then work on the psychological level without the underlying doubt that the problem lies elsewhere.<\/p>\n\n<h2>What to do during a nighttime attack<\/h2>\n<p><strong>Turn on a soft light.<\/strong> Darkness fuels disorientation. You don&#8217;t need to turn everything on: it&#8217;s enough to make out the outlines of the room.<\/p>\n<p><strong>Don&#8217;t get up abruptly.<\/strong> Sitting on the edge of the bed is enough and doesn&#8217;t add to the activation.<\/p>\n<p><strong>Lengthen your exhale.<\/strong> Don&#8217;t breathe deeply and quickly, as this worsens hyperventilation. Inhale counting to four, exhale counting to six or eight. It&#8217;s the long exhale that activates the parasympathetic nervous system and slows down the heartbeat.<\/p>\n<p><strong>Anchor yourself to something concrete.<\/strong> Place your feet on the floor, feel the ground. Mentally name five things you see in the room. This helps bring attention back outside the body.<\/p>\n<p><strong>Remember that it will pass.<\/strong> The peak lasts a few minutes. Telling yourself \u201cthis is a panic attack, I&#8217;m not dying\u201d reduces the secondary alarm, which is what fuels the escalation.<\/p>\n<p><strong>What to avoid:<\/strong> constantly checking your pulse, searching for symptoms online in the middle of the episode, waking someone up to be reassured every time. In the short term these calm you down, but in the medium term they reinforce the problem.<\/p>\n\n<h2>Why they happen<\/h2>\n<p>The factors are the same as for daytime attacks \u2014 an underlying vulnerability, a period of stress that acts as a trigger, and the mechanisms that then maintain the problem \u2014 with the added difficulty that at night the defenses and controls that keep activation in check during the day are absent. You start to fear the moment of falling asleep, you put off bedtime, you wake up several times to check that everything's okay. The result is that you sleep less and worse.<\/p>\n<p>And <strong>sleep deprivation lowers the activation threshold<\/strong>: an organism that has slept little is more reactive, more easily alarmed. This makes a new episode more likely, not less.<\/p>\n<p>It's a self-sustaining cycle, and for this reason it rarely resolves on its own by waiting.<\/p>\n\n<h2>How to intervene<\/h2>\n<p><strong>On sleep.<\/strong> Regular sleep and wake times, reduced caffeine and alcohol in the second part of the day, no screens in the last hour. These are simple and remarkably effective indications, precisely because they act on the threshold.<\/p>\n<p><strong>On physiological regulation.<\/strong> The <strong>Autogenic training<\/strong> teaches how to actively modulate the body's activation, and lends itself particularly well to this scenario: it's practiced before sleeping and becomes a tool available even in the middle of the night.<\/p>\n<p><strong>On the mechanisms that maintain it.<\/strong> Cognitive-behavioral work intervenes on the catastrophic interpretation of bodily symptoms and on the control and avoidance behaviors that reinforce the cycle.<\/p>\n<p><strong>On what lies beneath.<\/strong> Nighttime attacks often appear during periods of prolonged stress or after events that haven't been processed. When traumatic experiences are at the root, I consider working with<a href=\"https:\/\/martinafino.it\/en\/emdr-firenze\/\">EMDR<\/a>; Gestalt work on awareness helps recognize what that alarm is signaling.<\/p>\n\n<h2>When to ask for help<\/h2>\n<p>It's worth turning to a professional when the episodes recur, when you start fearing bedtime, when sleep quality steadily worsens, or when your day is affected. There's no need to wait until they become frequent: the earlier you intervene on the cycle, the easier it is to break it.<\/p>\n<p>If thoughts of harming yourself or no longer wanting to exist appear, don't wait for an appointment: talk about it immediately with your doctor, go to the emergency room, or call the <strong>112<\/strong>.<\/p>\n\n<h2>Frequently Asked Questions<\/h2>\n\n<h3>Can you die from a nighttime panic attack?<\/h3>\n<p>No. However intense the symptoms and real the fear, a panic attack is not life-threatening and resolves on its own. It remains important, at the first episodes, to have a doctor rule out organic causes that can resemble it.<\/p>\n\n<h3>How long does a nighttime panic attack last?<\/h3>\n<p>The peak arrives within a few minutes and the episode generally resolves within ten to twenty minutes. The tail of physical activation \u2014 trembling, difficulty falling back asleep \u2014 can last longer.<\/p>\n\n<h3>Why do they come specifically at night?<\/h3>\n<p>There isn't a single answer. At night, the distractions and control mechanisms that keep activation in check during the day diminish; moreover, some sleep phases involve physiological variations that can be interpreted by the body as alarm signals.<\/p>\n\n<h3>Do I need to sleep with the light on?<\/h3>\n<p>In the short term it can be reassuring, but if it becomes a necessary condition for falling asleep it risks turning into a safety behavior, that is, one of those habits that maintain the problem in the long run. Better a light that's quickly accessible than one that's always on.<\/p>\n\n<h3>Are nighttime attacks more severe than daytime ones?<\/h3>\n<p>Not more severe, but more disorienting, and with a greater impact because they affect sleep.
